- Submit the APPLICATION FORM together with FORM 137 or 138/ High School Report Card or OFFICIAL TRANSCRIPT OF RECORDS if College Student or Graduate.
- Include (1) 1×1 picture and typed-written TESTIMONY of your conversion and calling as well.
- Applicants also need to secure a PASTOR, AN ELDER, and A CHRISTIAN FRIEND’S RECOMMENDATION LETTER (recommendation letters are to be given by the REGISTRAR)
